Overview
Create forms in CollabOS, post a panel to Discord, and let members complete native Discord modals. Submissions are stored in CollabOS and answers can also be delivered to a response channel you choose. There is no Forms slash command or external form URL required for this flow.
Before you begin
You need an active OWNER, ADMIN, LEAD or MANAGER membership and Community & Discord or Everything access. Connect the workspace’s Discord server first. In both configured channels, the bot needs View Channel, Send Messages and Embed Links. Use a private staff response channel if answers may contain sensitive information.
Create a form
- Open Community & Discord > Forms, select the Forms tab, and choose Create form. The adjacent Wallet Collections tab is a separate wallet-address collection workflow; switching to it hides Create form.
- Enter an internal form name so your team can identify it.
- Set the Discord title, description and open-button label that members will see.
- Choose the post channel for the member-facing panel.
- Choose the response channel for answers. This can be different from the post channel.
- Set Enabled, add your questions and save. Use Preview to review the saved form before posting.
Customize the opening button
- Create a form or open Edit on a saved form. Beside Open button label, set Button emoji (optional) if you want one. Use Unicode or a supported custom Discord emoji such as <:form:123456789012345678>.
- Choose Button display: Text Only, Emoji + Text or Emoji Only. Emoji Only requires a valid emoji. Clearing the emoji while using Emoji Only switches the editor back to Text Only.
- Choose Button style: Primary — Blurple, Secondary — Gray, Success — Green or Danger — Red. Review Opening button preview.
- Click Save form. Existing forms default to their saved label, no emoji, Text Only and Primary; an empty editor label is saved as Open Form.
- On a published form, click Refresh / repost to apply the saved button appearance. Saving alone does not update the Discord panel or publish an unpublished form.
Questions
A form supports 1–5 questions in one Discord modal. Choose Short text or Paragraph for each question, add a question label and optional placeholder, and select required or optional. Set a minimum length when needed and a maximum length from 1 to 4,000 characters; the minimum defaults to zero and cannot exceed the maximum. An optional question may be left blank.
Titles and question labels allow up to 45 characters; placeholders allow 100. Use Move up to reorder questions and Remove question to remove one from the current version. Keep at least one question and no more than five.
Post or refresh a form
Choose Post form to publish the saved panel. Members click its button to open the native modal. CollabOS tracks the Discord message so Refresh / repost updates that message; a confirmed deleted message can be replaced. Permission, network and other unconfirmed edit failures return an error instead of silently posting another copy.
After editing panel copy, use Refresh / repost to update the visible Discord message. Changing the post channel immediately invalidates the old tracked panel and best-effort removes its buttons. Save the new channel, then Post form to create and track a new message there. If Discord cannot remove the old button, that stale button is still rejected.
You can change the response channel by editing the form. New submissions use the currently configured response channel.
Receive and review submissions
Members receive a private acknowledgement: Form submitted — Thanks, your response has been received. Answers are saved in CollabOS before delivery to the response channel. Delivery failure does not lose the submission or expose it publicly.
Open View submissions to read saved answers and their submission times. Filter by date or delivery status: PENDING, DELIVERED or FAILED, and use Load more for older results. Pending or failed results remain readable in CollabOS. Check the connection and channel permissions if delivery fails; automatic delivery retries are not provided.
Response messages identify the submitting member and show the questions, answers and timestamp. Submitted text cannot trigger uncontrolled Discord mentions.
Editing and disabling
Editing questions creates a new version and does not rewrite old responses. Historical answers keep the original question labels. Disabling prevents new form interactions without deleting submissions; you can still review the stored history. Re-enable and repost when you want to accept responses again.
